(2018)Målet med studien är att undersöka blodtryckets betydelse vid cerebral småkärlssjukdom hos typ 1-diabetiker. Sambandet har tidigare varit okänt, men i en studie av FinnDiane verkar högre systoliskt blodtryck vara av betydelse. Typ 1-diabetes är associerat med sjukdomar i hjärnans blodkärl, varav cerebral småkärlssjukdom är ett tillstånd där hjärnans små blodkärl skadas. Tecken på skada kan ses genom avbildning med magnetisk resonanstomografi (MRI). För denna studie undersöktes 73 individer med typ 1-diabetes ur FinnDiane-studien. De undersökta genomgick MRI av hjärnan och 24-timmars blodtrycksuppföljning. Systoliskt och diastoliskt blodtryck, medel arteriellt tryck och pulstryck under dygnets olika perioder registrerades och mätresultaten analyserades i relation till MRI-fynd. I denna studie hade systoliska och diastoliska blodtrycket nattetid samt medel arteriellt tryck nattetid ett signifikant samband med cerebral småkärlssjukdom. Hypertoni nattetid höjde oddsen för cerebral småkärlssjukdom fyrfalt. Av de undersökta hade 30 % maskerad hypertoni (normalt mottagningsblodtryck men förhöjt tryck i dygnsuppföljning) och förekomsten var högre bland individer med cerebral småkärlssjukdom. Slutsatsen är att cerebral småkärlssjukdom verkar ha ett samband med högt blodtryck nattetid samt med maskerad hypertoni hos individer med typ 1-diabetes.

(2021)Physical fitness has declined during the last decades in adolescents. Furthermore, several studies have found a positive association between physical fitness and brain volume in adolescents, which is noteworthy since the adolescent brain undergoes substantial changes during growth and maturation. However, despite the importance of the cerebellum on adolescents' cognition and coordination, there remains a paucity of evidence on the associations between physical fitness and cerebellum characteristics. Thus, a cross-sectional approach was used to explore the relationship of cardiorespiratory fitness (CRF), power, speed-agility, coordination and overall neuromuscular performance index (NPI) with total gray matter (GM) volume of the cerebellum as well as lobules VI & VIIb, and crus I volume in 40 (22 girls; 18 boys) adolescents. Peak oxygen uptake (V̇O2peak) was measured by the maximal ramp test on a cycle ergometer, lower limb power was determined with standing long jump (SLJ), speed-agility was assessed with the 10 x 5-m shuttle-run test, upper limb coordination was determined with the Box and Block Test (BBT) and NPI was calculated as the sum of SLJ, BBT and shuttle-run z-values. Lean mass (LM) and body fat percentage (BF%) were measured using a bioelectrical impedance analysis. Cerebellum GM volume, lobules VI & VIIb, and crus I volumes were measured by magnetic resonance imaging (MRI). Results demonstrated that V̇O2peak/LM was negatively associated (β = -.045 P= .014) with cerebellum GM volume. No statistically significant associations were found between SLJ, shuttle-run, BBT scores or NPI and cerebellum characteristics in all participants. However, a poorer shuttle-run time was associated (β = -.363 P = .024) with smaller crus I volume in girls and V̇O2peak/LM was negatively associated (β = -.501 P = .031) with lobule VIIb volume in boys. These findings suggest that, in general, CRF and speed agility are associated with cerebellum characteristics in adolescents and there may be sex differences. The results extend our knowledge of the associations between physical fitness and brain volume, but more studies should be conducted to understand the relationship further.

(2019)Schizophrenia is a severe and often chronic disease, which causes substantial harm both to the individual and to the whole society by large. Schizophrenia has long been associated with widespread anatomical alterations in the brain, with the most extensive changes occurring during the first psychosis. More studies have also increasingly focused on whether any alterations are already noticeable before the psychosis. Knowing this enable to both start the treatment earlier, thereby improving the prognosis, and to develop novel, preventive treatments. The articles included in this review consist of longitudinal magnetic resonance imaging studies which compare the brain anatomy of inviduals with a high-risk of schizophrenia making the transition to psychosis or not. The review includes both genetic and clinical high-risk populations, with most of the articles included focusing on the latter. Taken together, the articles create a mostly consistent picture of the anatomical features of the brain associated with the transition to disease both before and during the psychosis. The amount of grey matter was found to be systemically lower and/or to reduce more rapidly than in normal population, especially in the prefrontal, parietal and temporal lobes, cerebellum and cingulate and insular cortexes. However, the whole brain volume was found to be increased. Also, the enlargement of the third ventricle was quite strongly associated with the transition to psychosis. Finally, the integrity and the amount of the white matter might have some connection to the transition, but the results are inconclusive. Based on the results, schizophrenia is associated with wide alterations in brain anatomy even before the first psychosis. It can be assumed that these alterations explain the symptoms of the disease, for example that the changes in the prefrontal cortex relate to the loss of cognitive ability. Better understanding the connection of brain anatomy to the symptoms and progress of schizophrenia enables more effective and earlier treatment.

(2020)Hankositeen proksimaaliosan vauriot ovat yleinen ontuman syy ratsuhevosilla. Vammoja esiintyy kaikenlaisilla hevosilla ikään, sukupuoleen ja käyttötarkoitukseen katsomatta. Vauriot johtuvat liiasta kuormituksesta hankositeeseen, joka voi olla seurausta liiallisesta tai vääränlaisesta rasituksesta, mutta usein vaurion syntymistä edesauttaa hevosen takajalan rakenteelliset asiat kuten suora kinnet ja vento vuohinen. Tässä kirjallisuuskatsauksessa keskitytään takajalkojen vammoihin, mutta vammoja esiintyy myös etujaloissa. Kirjallisuuskatsauksen tavoitteena on perehtyä hankositeen monimutkaiseen anatomiaan hieman syvemmin ja löytää yhtenäinen tapa diagnosoida ja hoitaa hankositeen yläkiinnityskohdan vammoja. Hankoside sijaitsee plantaarisesti sääriluun ja syvän koukistajajänteen välissä puikkoluiden ympäröimänä kiinnittyen sääriluun proksimoplantaaripäähän yläkiinnityskohdastaan. Vammojen vakavuuteen vaikuttaa hankositeen anatominen sijainti, koska mahdollinen vamma voi vaikuttaa myös sitä ympäröiviin kudoksiin; esimerkiksi avulsiomurtuma sääriluussa on mahdollinen löydös hankositeen yläkiinnityskohdan vamman yhteydessä. Yleensä ensimmäisenä oireena hevosella havaitaan ontuma tai suorituskyvyn alenema, mutta sen vakavuus vaihtelee. Mahdollinen ontuma usein pahenee rasituksessa. Muita kliinisiä oireita kuten turvotusta tai lämpöä havaitaan vain harvoin. Vammojen diagnosointi aloitetaan kuten minkä tahansa ontuman selvitys; ensin palpoidaan raajat huolellisesti, tarkastellaan hevosen liikkumista, tehdään tarvittavaksi todetut taivutuskokeet ja diagnostiset puudutukset. Epäiltäessa hankositeen yläkiinnityskohdan vammaa kuvantaminen aloitetaan yleensä ultraäänitutkimuksella. Röntgenkuvantamisella poissuljetaan mahdolliset luuvauriot vamman yhteydessä. Nykyään käytetään myös yhä enemmän magneettikuvantamista, jonka avulla pystytään kuvantamaan sekä pehmyt- että luukudos samanaikaisesti. Vamman diagnosoiminen vaatii kuitenkin kokemusta ja anatomian tarkkaa tuntemusta johtuen hankositeen monimutkaisesta anatomiasta. Hankoside sisältää kaksi lohkoa. Molempien lohkojen sisällä on pienempi juoste, jota ympäröi varsinainen jännekudos. Juoste sisältää löyhää sidekudosta, verisuonia, hermoja, lihaskudosta ja rasvaa. Erilaiset kudokset ja niiden määrän vaihtelu eri yksilöiden välillä aiheuttavat heterogeenisyyttä ultraäänikuvaan. Diagnoosiin pääsy ja hoidon aloitus varhaisessa vaiheessa parantaa ennustetta. Varhaisesta diagnoosista ja hoidosta huolimatta ennuste on kuitenkin varauksellinen. Yleisesti käytettyjen hoitojen tehosta on ristiriitaisia tuloksia, eikä yhtenäistä hoitoprotokollaa ole löydetty. Akuutissa tapauksessa hoito aloitetaan konservatiivisella hoidolla ja paranemista voidaan tukea lääkityksillä, esimerkiksi kortisonilla, PRP:lla ja kantasolu-hoidolla. Myös esimerkiksi shock wave-terapiaa on käytetty hankositeen yläkiinnityskohdan vammojen hoidossa. Hoidossa voidaan turvautua myös kirurgiaan, jolloin hankositeen yläkiinnityskohtaa hermottava syvä lateraalinen plantaarihermo katkaistaan. Hoitomuodosta riippumatta paraneminen kestää pitkään ja sen edistymistä tulee seurata ultraäänitutkimuksen avulla ennen liikunnan lisäämistä. Vammat myös usein kroonistuvat tai uusivat heikentäen ennustetta edelleen.
